Patient Financial Responsibility Statement
All patients are responsible for payment of office services at the time of service, including copayments, deductibles and any charges not covered by insurance. Please note the following points:
1. Patients are personally responsible for insurance deductibles, non-covered services, and any outstanding balances determined by your insurance provider after your claim has been submitted and reviewed.
2. We will submit claims to your insurance as a courtesy; however, any portion of the bill not paid by your insurance remains your responsibility.
3. Prompt payment of balances is appreciated to avoid disruption of care.
